This notice explains how LEAP Speech & Lactation, LLC may use and disclose protected health information and how clients may access this information. Please review it carefully.

Our Legal Duties

LEAP Speech & Lactation, LLC is required by law to maintain the privacy of protected health information, provide clients with this notice, follow the terms of this notice, and notify clients following a breach of unsecured protected health information.

We will not use or disclose protected health information except as described in this notice unless written authorization is provided.

Uses and Disclosures Without Authorization

We may use or disclose protected health information for treatment, payment, and healthcare operations. This may include providing or coordinating care, obtaining payment for services, completing administrative tasks, quality assurance, and compliance activities.

We may also disclose protected health information when required or permitted by law, including reporting abuse or neglect, public health activities, health oversight activities, legal proceedings, or to prevent a serious threat to health or safety.

Uses Requiring Written Authorization

Client Rights

Clients have the right to inspect and obtain a copy of records, request amendments, request confidential communication, request restrictions on certain disclosures, receive an accounting of disclosures, and obtain a paper copy of this notice.

Clients may also request that information not be disclosed to a health plan if services have been paid in full out-of-pocket.

Requests must be submitted in writing to the Privacy Officer.
